Welcome to
Creekstone Care Centre
We are a comfortable, modern Long Term Care Home that opened in April 2021. We are located in beautiful North Vancouver in the blossoming Lower Lynn Creek area with easy access to Phibbs Exchange and the Second Narrows Bridge. We focus on Quality of Life and embrace best practices in Person and Family-centered Care.
Our home offers community-style living with 2 houses per neighbourhood or floor. Each house has 15 bedrooms with its own dining area and TV lounge, as well as a bathing spa, library, and access to the outdoors. There are also common amenities including a bistro-events room, a hair salon, a family games lounge and upper floor patios to enjoy beautiful sunsets. We have underground parking and provide secure bike storage.
All bedrooms are fully private with ensuite washrooms and come furnished with an electric bed, wardrobe, dresser, night table and room chair. This is your home and we encourage you to personalize it with pictures, books, and ornaments. There are TV and telephone hook ups in each room, should you decide to purchase those services.
We are a licensed care home operating under the Community Care and Assisted Living Act. Safety is a priority and our quality is closely reviewed. Creekstone works with Vancouver Coastal Health as a contracted Long Term Care Provider and is part of BC’s Funded Healthcare System.
To live at Creekstone and/or for access to funded long term care, please contact Vancouver Coastal Health directly. Currently, we do not offer private pay long term care as the home is fully contracted for funded services.

The move to Creekstone will be a new experience for you, and for your family and friends, and we are here to help. The key for us is to get to know you, your story, and your interests so we can help you enjoy life, every day. We want to build on the connections and hobbies you’ve always had and introduce you to new friendships and experiences too.
Recreation and play is important at all ages, which is why it is considered therapeutic and a top priority for us. Our team delivers creative, diverse programs 7 days a week and helps grow new relationships. Our daily programs cover five domains including physical, intellectual, emotional, social, and spiritual. They are built from the interests of the people living at the home and accommodate all levels of ability.
Staying connected is also important to wellbeing and we openly encourage visits and welcome the involvement of family and friends. They can join in for recreation, for food and for watching the sunset from the patio. This is your home to enjoy.
Dining at Creekstone
Food adds enjoyment to life. We offer home-style meals that are freshly prepared on-site in our central kitchen under the direction of both a Chef and a Registered Dietitian.
Our dining services include daily meals and snacks, dietary supplements, diet modifications and assistive eating devices, if needed, to help maintain independence. We tailor our approach to meet your dining needs and we involve the Resident and Family Council for input on meals and the choices available.
Just like home traditions, food is a big part of our special celebrations including holidays, birthdays and cultural events. Residents enjoy eating in the private dining room of their own neighborhood while being served by a friendly and caring team. This provides comfort and opportunities to enjoy the company of others, including guests. We hope you will join us.
Our Care Services
Care is at the heart of what we do and we are specialists in aging and dementia needs. We provide 24-hour care through a team of professional nurses, health care aides, recreation and support service workers along with a Recreation Therapist, Dietitian, Social Worker, Occupational Therapist and Physiotherapy. Medical care is provided by visiting physician(s) along with an appointed Medical Director.
Our care services include Pharmacare B medications, incontinence products and basic mobility needs with additional items available for purchase. Options include dental services, hair care, telephone, internet, and other professionals such as massage or companion support.
Quality is closely monitored with reporting requirements under BC’s Community Care and Assisted Living Act. As a licensed home, regular and unscheduled inspections occur at least annually and the home is certified for quality by Accreditation Canada.

We are a knowledgeable connected team of nurses, health care aides, support service workers, managers, clinical and quality specialists, geriatric leaders and volunteers. Creating joy in the lives of seniors takes very special people with a passion for caring who have a drive for excellence and a deep commitment to make a difference.
We are always interested in expanding our team and we want to meet others who share a passion for values-based living. We are an inspired group who believe that together, as a connected community, we can make a difference far beyond the walls of the home. Our History
Creekstone Care Centre opened in April 2021 as a Long Term Care provider for BC’s Healthcare system. The location for the home was carefully chosen for its residential feel, friendly neighbourhood and easy access to main transportation links for commuting and connecting. The community around the home is evolving and offers modern urban-type living, a vision for a bustling town centre, on-street bike lanes and enhanced pedestrian crossings.
